TEAM COACHING QUESTIONNAIRE

To help everyone prepare for our meeting, please complete this questionnaire (focused on you and your colleague’s work together, your working relationship, and whatever else is most important to you), and skip any questions that don’t feel relevant. Some of the questions are intentionally open-ended; answer in the ways most relevant for you.

A note on privacy: I won't share what you've written directly with your colleague, but I may bring up the substance of what you've shared in our conversation. You're welcome to share your own answers with your colleague, if you choose.
